Thank you for volunteering at Avondale Elementary School! Please review the required steps below to complete your volunteer clearance.
Cost: $67 total
- $20 (fingerprinting – LMPD)
- $47 (DOJ processing)
Step 1: DOJ & FBI Clearance (Fingerprinting)
- Print and complete the Live Scan form and take it to a LMSV approved location.
- No appointment necessary; payment required
- Submit the completed form to the School Office Manager
Step 2: TB Clearance
- Submit a completed TB risk assessment from a healthcare provider, or
- Provide proof of TB clearance completed within the last 60 days
Step 3: Clearance Status
- Follow up with the School Office Manager regarding your clearance status
- Please note: processing may take several weeks
Important: Fingerprinting and TB clearance must be completed before you can begin volunteering.
