Online Enrollment – No physical visit to school is required. Enrollment is completed entirely online. Visit the district website at www.lmsvsd.org and click the “Enroll” tab. You will be directed to complete a questions portion, download a fillable PDF, complete and upload that document, upload required documents (immunization, birth certificate, 2 proofs of residency, physical and dental exams forms). Physical and dental may be turned in at a later date due to doctor office closures during COVID-19. Immunizations are still required.
