Welcome to the ESSPN Youth Sports League! We are thrilled to have your student join us for an exciting season of growth, teamwork, and fun. Please Note: Spaces are limited, and rosters are filled on a first-come, first-served basis. To help you stay on track and ensure a successful registration, we have broken the process down into three specific sections. Please complete each section below:
- Register the student on Eleyo (attendance tracking website). This step is required for us to manage daily attendance and ensure a safe, smooth experience for every student.
- Submit the Permission Slip Packet. You can access the packet below or pick up a hard copy from your school’s ESS program. Once completed, please return the packet directly to your school’s ESS Site Lead.
- Provide Proof of Medical Insurance. Please submit a copy of your insurance card or other proof of coverage. You may hand this in to your ESS Site Lead or email it to ESSPN@lmsvschools.org.
Note: If emailing, please include your School Name and Student Name in the subject line.
Eleyo Sign-up Information
Eleyo Account Setup
Create or log in to your Eleyo account: If you don’t have an Eleyo account, you will need to create an account. Have handy:
- Your student’s information: To register your student, the following information will need to be input into the system. Name, relationship to you, date of birth, current grade, and the school they attend.
- The Emergency Contact information of at least one other adult: In case of an emergency, we require at least one additional emergency contact to be listed and on file. You will need their Name, relationship to the student(s), address, and phone number.
Enrolling in ESSPN
After logging in, you will need to enroll your student in the ESSPN Sports League and create a new contract for this upcoming season. We’ve also created a step-by-step guide to help you through the process. Complete the enrollment process, which includes: adding your Child’s Information, adding at least one Emergency Contact, selecting your student(s) School & Sport, completing the Terms, Schedule, and Questions sections.
**ESSPN is a FREE sports program. You will never be charged**. If there is a charge on your account, please call our ESS offices at 619-644-3800.
Permission Slip Packets
Packet Details:
To prioritize the safety of all players, we require a fully completed permission slip packet before your student can participate. Please review the breakdown below to understand the forms included in this packet:
- Field Trip Permission Slip: This authorizes your child to travel by bus to and from away games.
- Medical Health Awareness: This section details and includes mandatory awareness and liability forms regarding Sudden Cardiac Arrest and Concussion/Head Injuries.
- Student-Athlete Contract: This is an agreement signed by the student, promising to uphold our team’s expectations and sportsmanship standards.
Packet Submission:
A limited number of printed packets is available for pick-up at your school’s ESS program. However, for faster access, we recommend using the downloadable options below. You have two ways to complete these:
- Print & Fill: Download the packet, print it at home, fill it out by hand, and drop it off at your school’s ESS site.
- Digital Fillable PDF: Download the packet to your device, type your information directly into the digital form, save the file, and email it to ESSPN@lmsvschools.org.
- Important: If you email your forms, you must include your School Site and Student Name in the email so we can file them correctly.
Insurance Information
Reason for Proof of Insurance:
Under state law, student-athletes must have confirmed healthcare coverage before they can compete in any extracurricular athletics. To comply with this requirement and ensure safety, we must have proof of insurance on file for every participant.
Acceptable Documents: Please submit a copy of one of the following items:
- Tri-Care Card: Important: You must completely black out all sensitive information on both sides of the copy before submitting.
- Valid Insurance Card
- Provider Letter or Email: A document from your healthcare provider that clearly shows the medical record number.
How to Submit: We only require a copy for our files. You can submit this in two ways:
- In-Person: Bring your document to your school’s ESS Program. Our staff can assist you in making a copy on-site.
- By Email: Take a clear photo or scan of the document and email it to ESSPN@lmsvschools.org
- Reminder: Please include your School Site and Student Name in the email so we can match it to your permission slip packet.
Confirmation Process
What Happens Next?
After you have completed the three steps above (Eleyo registration, Permission Slip, and Insurance), please allow 2–5 business days for our team to review your documents and approve your student’s contract.
How to Know You Are Approved
You will know your student is officially on the roster when you receive an email from ESSPN@lmsvschools.org with the subject line: “Welcome to the Spring 2026 Flag Football Season”. Important Note: Please check your spam or junk folders. If you have not received this specific welcome email, your student is NOT considered registered. Students may not attend practice until this confirmation has been received.
If it has been longer than 5 business days and you have not heard from us, please contact us immediately to check the status of your application.
